If you have lost your Club Permit Log Book you need to:
- obtain a Statutory Declaration form from VicRoads and fill it out accordingly to explain why the Log Book is required, and how many days were completed prior to the loss. It must be signed by the Permit holder.
- Contact the Club Secretary to issue you with a letter confirming you are a CIMAA club member, and requesting a replacement Log Book. It must be signed by a club signatory.
- Take the completed forms and your registration papers to VicRoads.
Alternatively, if you only need a Log Book without the registration label send a self-addressed envelope (which is big enough to contain a new log book) to the Club President together with $5, and you will receive a new log book by return mail.
